Professional Profile
Nurse Practitioner Sarah DiGeorge was inspired to advance her career through her work in critical care, where she saw how timely, high-quality care can make a real difference for patients and their families. That experience led her to become a nurse practitioner, allowing her to take on a greater role in patient care and long-term health management.
DiGeorge specializes in cardiovascular medicine, with a focus on heart failure, allowing her to care for patients both in and out of the hospital and providing continuity and support over time. She values the opportunity to improve patients’ quality of life and help patients stay out of the hospital whenever possible.
She cares for adult and older adult patients with a range of heart conditions, including heart failure, coronary artery disease, high blood pressure, arrhythmias, valvular disease and high cholesterol. Her work also includes medication management and remote monitoring to help patients better manage chronic conditions.
DiGeorge earned her Master of Science in Nursing from the University of Texas at Arlington and has extensive experience in both intensive care and cardiovascular care settings. She is a member of the Heart Failure Society of America and the American Association of Critical-Care Nurses.
Patients describe DiGeorge as thorough, dedicated, and genuinely caring. She focuses on preventing hospitalizations and strongly advocates for her patients’ needs. Many appreciate that they can reach out with questions and feel supported in managing their health.
During visits, patients can expect a straightforward and compassionate approach. She takes time to listen, explain conditions clearly, and review medications and lifestyle changes. The goal of her patient-focused care philosophy is for every patient to leave feeling informed, confident and supported. She works closely with patients, families, and care teams to create personalized plans that improve health, prevent hospitalizations and support long-term well-being.
The most rewarding part of DiGeorge’s work is building strong relationships with patients and families. She enjoys seeing patients improve and gain confidence in managing their health. When challenges arise, she feels honored to support patients and their loved ones through every stage of care.
Outside of work, DiGeorge enjoys spending time with her family and watching her children play sports. She also loves being outdoors, staying active, and enjoying time in the sun.
